- What is a typical day like for you—tell me what you do starting with getting up and ready for the day?
- What have you given up or had to change over the past 6 months/3 years?
- What would you like to do but is very hard for you to do right now?
- What would you like to be able to do better/more often/again?
- Tell me how you think of yourself 10 years ago and now
- What is your greatest fear or concern for you or your family about COPD?
- What about going to the hospital?
- What about a care home or living with family?
- What would you like me to put in your medical record for everyone to know about your wishes, fears and needs?
- Have you been concerned about dying? Do you have plans or wishes about dying such as where and what end of life treatment you want? Have you discussed this with your family and carers?
- Have you completed an Advance Care Directive or Living Will?
