Can Pulmonary Rehabilitation be delivered to people living with COPD through a web-based application in India?

Clinical Research Results Abstract Research Idea Abstract Research Question: Is it feasible to deliver Pulmonary Rehabilitation (PR) for COPD through a web-based application in India?Background: The need for PR in India is well established, considering the huge burden of chronic respiratory diseases. However, the uptake of PR is low due to barriers like unawareness of its benefits, travelling long distances to attend PR and vulnerability to COVID-19. To counter these issues, digital PR delivery has been recommended by the ATS/ERS and GOLD guidelines. In India, the pandemic led to an increased awareness of using technology in accessing healthcare services. As such, we are studying the feasibility of delivering digital PR in India.Possible methodology: Patient-centred methodologies including surveys, qualitative interviews and community engagement and involvement to develop digital modes for accessing PR. Patient-driven development of digital PR applications and its contents based on the needs of the end users are necessary. Feasibility and acceptability studies of the intervention. Randomized controlled trials to study effectiveness and efficacy of the digitally delivered PR will be imperative before these can be commissioned in clinical practice.Questions to discuss: Who will benefit from digital PR models based on factors like literacy, digital literacy, and internet/device availability? What will be the structure and content of the intervention to make it culturally appropriate? What will be the dependency of patients on their caregivers in accessing and utilizing digital PR? How will the intervention be monitored by medical staff? Which outcome measures will be more appropriate for the assessment of the intervention? Will the digital PR be acceptable, effective and efficacious in the management of COPD?
