EXTRA PULMONARY TUBERCULOSIS BURDEN IN HIGH ENDEMIC COUNTRIES

Clinical Research Results Abstract Clinical Research Results1.Aim: Tuberculosis, both pulmonary and Extra Pulmonary are still a major Health issue in the developing countries, involving Lungs and other systems of the body, presenting in different ways in the patients at the doctor’s clinic. It can disseminate leading to overwhelming infection and death. The rise in the number of cases of Extra Pulmonary Tuberculosis is either due to an increase incidence or due to an increase in the number of physicians who diagnose the disease in the society.2.Method: This survey was carried out at District TB Control office at Peshawar, a health facility for registration, providing free diagnostic and Anti TB Medicine, follow up of the Registered patients during a period of one year (1st Jan 2018 to 31st Dec 2018) diagnosed by Chest Radiography, Sputum microscopy, GeneXpert and Monteux Test. This Health facility also register all other cases of Extra Pulmonary Tuberculosis already diagnosed by family health Physician and Tertiary Care Hospitals having relative specialties and indoor diagnostic facilities. Results: Total 1202patients were enrolled, among which EP were 485 (32%)while pulmonary were 715 (68%). Among total of EP TB cases most cases were abdominal TB 179 (37%), pleural effusion 110 (23%) and Cervical lymphadenitis 111(23%). Extra Pulmonary Tuberculosis patients were registered for treatment as Two months intensive phase and four months continuation phase with in some cases longer period of Nine to one year in complicated cases e.g. Tuberculosis Meningitis, Pericardial Effusion and Abdominal Tuberculosis carried out open laparotomy and Biopsy result from the Pathologist.4.Conclusion: Majority of Extra Pulmonary TB cases were no bacteriologically confirmed, but the histopathology was showing chronic granulomatious infection compatible with tuberculosis supported by clinical examination, patient history, family history favoring Tuberculosis. Due to poor socioeconomic status and with limited technical resources availability we are unable to diagnosed evidence based extra pulmonary abdominal Tuberculosis.
